Health Advancement Program

Introduction:

Stowe Associates works with organizations to positively impact and manage the health and wellness of their employees and their associated healthcare costs. To assist those organizations, we are pleased to offer our emPower™ Health Advancement Program, a unique and comprehensive health management and worksite wellness program designed to assist employees in establishing a healthier lifestyle while increasing productivity and reducing total healthcare costs. Approach:

Initially, we will work with your team to establish appropriate goals and expectations for your group. We will help develop strategies from your past experience, and identify approaches for any communication and program aspects to make for a successful program. Our proprietary eCareConnect™ technology suite is the engine behind emPower (engaging, managing and producing outcomes with evidence-based results).

Tools:

  • Our Health Risk Assessment (HRA) classifies the 50 plus conditions we track into seven different levels, None, Some, Low, Moderate, Elevated, High, and Serious.
  • Our biometric and metabolic panel standard tests include height, weight, BMI, waist, hips, wrist, BP, Lipids, glucose, nicotine, GGT, and a metabolic panel. Scores are divided into five levels, Excellent, Good, Fair, Poor or Very Poor and each participant receives a WellCentives Score™ which reflects a percentage of possible points earned from the biometrics.
  • Claims Analysis may also be employed if claims are available. Using our Sweeper™ Claims Analytics program we can identify trends, perform case reviews and perform comparative analysis between plans.
  • Using this predictive modeling approach, we typically identity 20% to 40% of the population that is either previously diagnosed, at Elevated, High or Serious risk for a Predisease condition, or has a biometric score of Fair, Poor or Very Poor.
  • Each participant receives a custom configured online personal health management system, our Personal Health Desktop (PHD), based on the questionnaire and biometric/screening results.
  • Personal Health DesktopAlso included are four (4) Road to Wellness™ Lifestyle improvement programs, our interactive multi-step e-learning seminars for diet and nutrition, stress, smoking cessation and fitness.
  • A confidential Executive Summary Aggregate Report is prepared and presented to management that is based on the analysis of those health risks and behaviors determined by the collected data reports of your population.
  • Our Wholeistic HealtheCoaching program focuses on the whole person. Our wellness coaching program offers each and every participant an opportunity to focus on their physical and mental well-being – including disease or condition management, weight, fitness, nutrition and stress, and the mindset to sustain wellness.
  • Also included in our HealtheCoaching program is our full healthcare advocacy service. This means that any participant or their family member can call or come to the onsite health room to ask questions about care they or their family received. Our coaches can explain treatments; provide guidance and interpretation for participants that have questions.


Health Counts™ Participation Incentive Points Program:

Health Counts™ is an effective incentive tracking and management tool that is essential to keeping your employees motivated and engaged in healthful behaviors. By creating campaigns and activities for your group, you can motivate participants by awarding points. This ensures that you can track your progress of group participation. Studies show that by rewarding engaged employees, you improve participation and improve outcomes.
